Disaster Relief Form for the Request of Funds

This is an online version of the Disaster Relief Fund Request form that pastors can use to request funds for their disaster affected churches and communities.

  • Please fill out the form below to request disaster relief funds. If you have questions about how these funds can be spent, please email Kelly Crawford at kcrawford@nswactbaptists.org.au or call on 02 9868 9257. Funds are generally spent on food, fuel, utilities, mental and physical health needs, etc. We discourage giving funds away as cash and roughly limit the amount spent per family unit to $1000. Each church receiving funding will need to acquit these funds using a separate form that will be provided. A summary of how the funds were spent including any supporting documentation e.g. invoices will need to be submitted.
