The Association strongly encourages all pastors to engage in some form of regular Guided Reflective Practice: Professional Pastoral Supervision, Mentoring, Coaching and Spiritual Direction are key types of Guided Reflective Practice. Engaging in 1-2 of these practices in any given season is considered healthy practice. Please note that you may choose to use any Guided Reflective Practice to contribute to your hours in the Personal/Spiritual Formation section of this form below.

To complete your CMD Report by reference, you are required to submit 5 references which, together, attest to your ongoing personal/spiritual development, professional/academic development and your denomination involvement.

The following CMD reporting option is available to chaplains, mission agency staff and Baptist agency staff outside of pastoral ministry (e.g. Morling College Staff). All three statements are required for CMD completion.

To complete your CMD report using formation activities, you are required to list and reflect on 10 hours of activities during the reporting period, for each of the three formation areas (totalling 30 hours of activities). For Retired Ministers with Ongoing Accreditation, the hour requirement is halved to 5 hours per area (totalling 15 hours of activities).

Learning Goals/Outcomes: To develop and facilitate transformation spiritually as a pastor/minister. Activities could include, but are not limited to, such things as: Involvement with a mentor/spiritual director/counsellor/therapist; Reflection on current ministry role; Spiritual/Family/Individual retreats; Pastoral renewal group; Personal reading in spiritual formation; Spiritual formation courses; Short-term mission trip; Marriage Enrichment course. Approx 500 words in total required in reflection for this whole section.

Learning Goals/Outcomes: Enable improvement vocationally and professionally as a pastor/minister.Activities could include, but are not limited to, such things as: Attendance at seminars, courses, conferences; Post-graduate or distance studies; Professional reading & reflecting on academic learning; Involvement in a professional area or body; Involvement in pastoral/church reviews, observations or evaluations; Write an article for a journal; Present a paper at a conference; Teaching and/or designing workshop/lectures (if not primary ministry role).Approx 500 words in total required in reflection for this whole section.

Learning Goals/Outcomes: Extend network connection and relationship with those in the Baptist Association of Churches.Activities could include, but are not limited to, such things as: Attendance at Assemblies and The Gathering; Membership and involvement on a denominational committee; Involvement in regional pastors networks; Involvement in Regional Association meetings; Involvement with Church Consultations; Involvement in training others for ministry; Mentoring/Coaching a student or church leader; Outside main ministry involvement (eg., chaplaincy, community work, schools).Approx 500 words in total required in reflection for this whole section.

This is space for you to listen to God and consider where He is calling you to grow and learn and invest in the years ahead. This is not designed to be prescriptive, but to encourage forward things. Writing these goals according to SMART principles is encouraged but not required (Specific, Measurable, Achievable, Realistic, Timely).
