Form 2024

9^th October - 11^th October, 2024

FORM is a place for church leaders, pastors, members of church communities, small groups, and anyone wanting to learn how to better engage missionally with their communities, families, colleagues and friends.

FORM is focused on the formation and development of your skills as well as a support network for you that will help make sharing stories about Jesus powerful and effective.

If you are thinking of planting a church, starting a new congregation, sharing your faith or would just like to know more about engaging missionally with your world, then FORM is for you.

This year FORM will revolve around the theme SEEK FIRST.

FORM will be an experience for the whole family. It will cater for multicultural church expressions.

Providing an opportunity for everyone to hear from God with times where all generations will gather together as well as age-specific experiences.

Translation will be provided for several languages.Places will fill fast so be sure to register early!

I acknowledge that camp activities can be hazardous, and that the attendee participates at their own risk. I understand the camp leaders take reasonable steps to provide a safe environment for the attendee and to ensure all equipment supplied by them for any activities is of a reasonable standard.

I acknowledge the Baptist Association of NSW/ACT, its staff and volunteers will not be liable for any injury that may be suffered by this attendee, which arises either directly or indirectly from, or in connection with, FORM 2024.

I hereby agree to indemnify Association of Baptist Churches of NSW/ACT, and any other Baptist churches/organisations against any and all claims arising from, or in connection with, any injury that may be suffered by this attendee, or that the attendee may cause to another person, as well as any loss or damage to property, equipment or personal effects belonging to this attendee, or any other person, arising either directly or indirectly out of, or in connection with, FORM 2024.

Refund/Cancellation Policy
The Baptist Association of NSW/ACT understands circumstances change and offer the following alternatives to assist you should you need to alter your registration. Cancellations and refund of payments can be arranged by writing to The Baptist Association of NSW/ACT before 25th September 2024. Any cancellations before the 25th September 2024 incur a cancelation fee of $80 per person. No refunds will be issued after this date. The Baptist Association of NSW/ACT can allow for special consideration of your circumstances in regard to this policy. If this is the case, please contact The Baptist Association of NSW/ACT to discuss your circumstances.

Privacy Statement
How we collect, hold, use and disclose information about you is important to us. The purpose of collection, holding, use and disclosure of the information is to fulfil the ministry and administrative functions of the Association. We also collect your information to administer your registration for events and invite you to events. If you do not provide your information, we may not be able to do this. We may communicate with you for these purposes by email or mail, which you can unsubscribe to at any time by writing to us. We may share our information collected with other Baptist Churches, affiliated organisations, and hosts of events you are attending. If you believe a correction is required, please contact us. By submitting this form, you consent to us using and disclosing your personal information and contacting you as described above. Photography/Video Agreement
As part of attendance at FORM 2024, you and your child may have your photo/video taken. These photos and videos are stored by Association of Baptist Churches NSW/ACT (in accordance with the Privacy Act of NSW). Video footage and photographs will be taken to record children’s events and may be used to promote FORM and the Association of Baptist Churches of NSW/ACT in both print and digital formats, including a website. Your child will not be identified in these photos by name. You may withdraw this permission at any time by writing a letter to the Communication and Events team, Level 4, 5 Saunders Close Macquarie Park NSW 2113, or via email.
